Buddy Day 2025 Permission Slip

Dear Feeder School Fifth Grade Parent or Guardian,

Your child has been cordially invited to attend an articulation day at Frank D. Paulo Intermediate School 75. This activity is known as “Buddy Day.” Your child will have the opportunity to participate in a variety of fun and educational activities, and will also attend classes with a current sixth grade "buddy" assigned to your child.

Here are the details of the visit:

Date: Tuesday, May 27, 2025
Arrival Time: 7:45AM, via Huguenot Avenue main entrance
Release Time: 1:45PM, via Huguenot Avenue main entrance
Location: I.S. 75, 455 Huguenot Avenue, 10312
Transportation: parent/guardian must drop off and pick up
Lunch: provided* (pizza and a beverage)
Purpose of Trip: Incoming Sixth Grade Articulation (Buddy Day)
Specific Clothing/Equipment required for this trip: N/A

*If your child cannot eat or does not want pizza, please send them with lunch.

Upon the completion of our activities, parents/guardians will pick up their children from our auditorium. Please use the main entrance and have a photo I.D. with you when you arrive.

b) I agree that in the event of an emergency injury or illness, the staff member(s) in charge of the visit may act on my behalf and at my expense in obtaining medical treatment for my child.

c) I understand that my child is expected to behave responsibly and to follow the school’s discipline code and policies.

d) I agree and understand that I am responsible for the actions of my child. I release the school from all claims and liability that arise in connection with the trip, except if due to the negligence of school officials.

f) I understand that alcoholic beverages and/or illegal drugs are prohibited and have discussed this prohibition with my child. I understand that if my child is found in possession of these substances, he/she will be subject to school disciplinary procedures and possible criminal prosecution.

g) I understand that students who violate the school's discipline code may be excluded in the future by the school from participating in a trip.
